Instructions for completing the financial aid application (en Espanol haga clic aqui):

FINANCIAL AID APPLICATION
FORMULARIO DE AYUDA FINANCIERA 
(EN ESPANOL)

1. Read the front page of the application and
make copies of the requested documents.
2. Read the directions and complete the financial assistance application.  Do not leave any line blank.  Put a "0" or "N/A" if that line doesn't apply to you.
3. Sign the back of the application.
4. Attach your 2008 tax forms, W-2's, and other required documentation to the application.  Include the $21 check or money order made payable to PSAS.
5. Mail the application and everything included on line 4 to:
           Private School Aid Service
           P.O. Box 770728
           Lakewood, OH  44107-0034
6. Call the Tuition Office at 356-6377 with  questions.
